School Sports Physicals

All area students are invited!
It is time for school sports
physicals and we are pleased to be able to offer you our care. We would like to make it an efficient and
stress free experience for each student.
We have listed some information below that we hope will be helpful to
you.
1. These are sports
physicals for students who will be in 7th grade
through 12th grade in the 2008/2009 school year.
2. Parents are welcome to participate in the process, but
it is not required for them to be present as long as they have signed their
portion of the sports physical form.
3. Students will be seen on a first come, first serve
basis.
4. Cost is $20.00 to be paid at the time of check-in.
5. The student/guardian portion of the sports physical
form should be completed and hand carried to the office at the time of check-in
to expedite the process. These forms can
be picked up at the school or the clinic.
6. A urine sample will be collected for this evaluation.
7. You will not have to take your clothes off for this
exam. However, boys will be checked for
hernias so please dress appropriately.
8. After the exam, you will be given your signed form if
everything is normal. If any abnormal results
are noted, the healthcare provider will talk to you about it.
